A Program Guide for IOP and PHP Directors

# Adding Hypnotherapy to IOP and PHP Programs

**Short answer:** In IOP and PHP programs, hypnotherapy works best as a scheduled complement to counseling and group therapy, aimed at stress, sleep and self-regulation. Programs add it by hiring, training staff, contracting a practitioner, or using an on demand telehealth service, and should screen clients and coordinate sessions with the clinical team.

## IOP and PHP at a glance

Under the ASAM Criteria (3rd edition), Level 2.1 intensive outpatient care runs 9 to 19 hours per week for adults, and Level 2.5 partial hospitalization runs 20 or more hours per week. The 4th edition, released in 2023, renamed Level 2.5 "High Intensity Outpatient." SAMHSA describes core IOP services as individual counseling, group therapy, family psychoeducation and case management.

Those schedules leave room for complementary services that support what counseling is already doing.

## What hypnotherapy can and can't do

The research is strongest for pain and procedure-related distress, and promising for anxiety and sleep. A 2019 meta-analysis found hypnosis reduced anxiety, with better results when combined with other psychological treatment. Evidence for substance use outcomes specifically is limited: a 2019 Cochrane review of 14 trials found no clear advantage over behavioral support for smoking cessation, at low certainty.

That points to a clear role: hypnotherapy as an added support for stress, sleep and self-regulation inside the program, not a replacement for evidence-based addiction treatment.

## Where it fits in the week

- **IOP:** one weekly session, individual or group, scheduled around counseling.
- **PHP:** a recurring group session plus individual sessions for clients the team selects.
- **Step-down:** continuing sessions as clients move from PHP to IOP to outpatient care, for continuity.

## Staffing options

| Option | Best for | Trade-off |
| --- | --- | --- |
| Hire a clinician with hypnosis training | High, steady demand | Fixed cost and recruiting time |
| Train licensed staff | Teams that want an internal skill | Months before staff are ready |
| Contract a local practitioner | Small programs with local talent | Your team vets and schedules |
| On demand telehealth service | Variable demand, fast launch | Sessions are remote |

## Safety and coordination

- Screen first. Mayo Clinic and the NHS caution against hypnosis for some people with severe mental illness or a history of psychosis.
- Avoid memory-recovery work. Hypnosis does not improve accurate recall and can increase false memories.
- Agree how session notes reach the treatment team and who the client contacts with concerns.

## Where Sybilry fits

Sybilry connects behavioral health and wellness programs with vetted hypnotherapists for on demand telehealth sessions, one-on-one or in groups. Every hypnotherapist in the network is independently credentialed and vetted for behavioral health and co-occurring experience, and a Sybilry account rep coordinates every session. Programs pay only for the sessions they schedule, with no setup fee, retainer, minimum volume or contract, and no added fee to the client.

## Frequently asked questions

### How many hours per week is IOP versus PHP?

Under the ASAM Criteria (3rd edition), adult IOP is 9 to 19 hours per week and PHP is 20 or more hours per week. SAMHSA describes IOP as at least 9 hours per week for adults.

### Is hypnotherapy an evidence-based treatment for addiction?

Not on its own. Evidence for substance use outcomes is limited, while evidence for anxiety, sleep and pain is stronger. It fits best as a complement to evidence-based addiction treatment.

### Can PHP programs offer group hypnotherapy?

Yes. Group sessions can teach relaxation and self-regulation skills to several clients at once, and the largest head-to-head trial, in irritable bowel syndrome, found group hypnotherapy not inferior to individual sessions.
